I downloaded some DEM data cdem_dem_031E.tif, a 90 MB file. After trimming to a smaller area of interest I saved cdem_dem_031E_t_HLBRR.dem only 631 KB.
I then opened a route file that covers the area, road_segment_1_3.shp 63.9 MB. A few roads are shown on screen, the rest are off screen. When I attempt to save a road_segment_1_3.str file, it doesn't seem to stop writing to the file.

Isn't it only supposed to be writing those road segments that are in the trimmed DEM file? How do I tell it to ignore off screen roads? Or is there something else going on?

No, there is no automatic trimming. A shape file of 64 MB will be quite a huge thing. .shp is a binary format, .str is text. TransDEM will try to write it all. It will take a while and result will be a very large file.
