Is it possible in the newest release of TransDEM to change the elevation of an existing route in Trainz 2019? I tried using the process as I remember it (opening the route up for edit and then selecting the editing folder) but it doesn't show that the route is there. Thanks!

Unfortunately, it's not possible since T:ANE SP2, because of the significant Trainz route format changes I never got around to decode.

I still have the route I need to adjust in TANE however my install is SP4. I wonder if I could uninstall and then install TANE up to SP2, adjust the route and then import into 2019?

I don't think it's possible as you cannot save your route to an older file format in Surveyor. You can easily tell. If you route has many .gnd, .obs files etc instead of a single one for each type, you are in the per-baseboard file structure, which makes out some of the breaking changes.
